Welcome to the NPHealthCheck form, which we use to assess your organization and apply rankings based on the five different indicators.
Please note that the form you’re about to complete may take up to 30 minutes to complete, mostly based on the availability of information. We recommend that, before you begin the survey, you gather the following:
- most recent Form990 or year-end financials
- board policies and procedures
- board list
It’s also important to know that the ratings are scored, in part, by the answers you submit with each question. Because we take great pride in accuracy, our verification process is rather detailed and may result in further communication with you for additional information. In fact, we request that you email us the following files upon completion of this form:
- most recent Form990
- Gift Acceptance policy
- Board Conflict of Interest policy
- Annual Report (if one exists)
